Number 4 is when they just put in the trash! It's like it never came. Number 5 , i find, I'm always having to train my staff to not respond to clients unless they've provided us a written copy. If you call your accountant in a tizzy wanting an immediate explanation for the notice, you're not going to get it, unless you provide a written copy of the notice, period.
